This Privacy Policy is not applicable to the information collected, used, or disclosed in connection with your telemedicine visit or any other healthcare or treatment-related services you may receive from our affiliates, including the BalanceTLC affiliated professional practices and the BalanceTLC Pharmacy. Once you become a patient of our affiliated professional practices for medical or mental health services, use the BalanceTLC Pharmacy services, or use the services of any member of our Organized Health Care Arrangement, any Personal Information you have provided through your account while using the Services will become subject to these affiliates’ joint Notice of Privacy Practices.

BalanceTLC (“the Company,” “we,” or “us”) wants you to be familiar with how we collect, use, and disclose information. This Privacy Policy outlines our practices in connection with the information we collect through:

  • Websites that operate symptom checkers and from which you access this Privacy Policy (the “Websites”);
  • Software applications of symptom checkers that we make available for use on computers and mobile devices (the “Apps”);
  • Our social media pages and apps (collectively, our “Social Media Pages”);
  • HTML-formatted email messages that we send to you that link to this Privacy Policy or other communications with you; and
  • Offline business interactions you have with us.

We collectively refer to the Websites, Apps, Social Media Pages, emails, and offline business interactions as the “Services.”

What Types of Information Do We Collect?

Personal Information: BalanceTLC and its service providers collect Personal Information in various ways, including:

  • When you use the symptoms checker, Personal Information is collected, such as your name, phone number, date of birth, gender, location, IP address, billing information, email address, and, in some cases, a copy of your driver’s license for identity verification purposes.
  • If you become a BalanceTLC patient through telemedicine, tele-mental health, or pharmacy services, any Personal Information you provide, as well as health-related information, will be subject to the affiliates’ joint Notice of Privacy Practices and not this Privacy Policy.

Other Information: BalanceTLC collects non-Personal Information, such as technical and aggregated usage information, about users who use the service anonymously or have an account. This information includes browsing activities, browser information, app usage data, and information collected through technologies such as cookies and pixel tags.

Sponsor Organizations: If an organization such as your employer, university, or health plan is paying for access to BalanceTLC services on your behalf, you may need to provide additional registration information to confirm eligibility. This information may include your name, email address, date of birth, and phone number. The information is required to confirm program or benefit eligibility and prevent insurance and benefit fraud. Your Sponsor Organization or the subscriber of your benefits will not be provided with this information if you have already given them similar information in your employee file or record, unless you are a family member of an eligible employee.

Collection of Personal Information

BalanceTLC collects information from various sources to provide its services to users. The information collected falls into three categories: information provided directly by users, information collected automatically, and information received from third-party sources and social media platforms. If users do not provide the requested information, BalanceTLC may not be able to provide its services.
